Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Please inform us of any changes to your insurance coverage.
Your estimated patient portion must be paid at the time of service. As a service to our patients, we will bill insurance companies for services. If claims are not paid within 60 days, you are responsible for the balance. If you have any questions, our courteous staff is always available to answer them.
Affordable Financing Options
Don’t let finances stand in your way of receiving the quality care you deserve! We are pleased to offer financing options with a $0 down payment and low monthly payments that make your treatment more affordable. We also accept third-party financing through CareCredit. These flexible payment options allow you to pay for your treatment over a period of time so you can focus on what matters most – staying healthy!
Our office staff will be glad to assist you with your insurance coverage. When you first visit our office, bring your current insurance card with you. If you change insurance companies or join another employer’s plan, please be sure to let our insurance department know. If an insurance referral is necessary, please bring the completed form with you to your appointment.
Dougal, McClellan & Sullivan Eye Associates is in-network/a preferred provider for all major insurances, including BCBS, UnitedHealthcare, Medicare and Medicaid.
We will submit all insurance claims for you. We will fully attempt to help you receive full insurance benefits; however, you are personally responsible for your account, and we encourage you to contact us if your policy has not paid within 45 days. If we are not contracted under your insurance, you will be given a copy of services and charges to file with your insurance claim form.
